Job Description:
- Identify Patients that need special medical and education needs/management.
- Conducts assessments of family members of regular component Air Forse sponsors to determine if special medical and/or educational conditions exist that require Exceptional Family Member Program enrollment.
- Comprehensively collects patient information and medical status, and continued evaluation of an established care plan.
- Collaborates with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care.
- Shall be integrally involved in the Family Member Relocation Clearance process.
- Demonstrate a working knowledge of professional standards and ethics regarding the delivery of clinical social work services.
- Ensures that sponsors are provided all necessary documents for enrollment in TRICARE, Extended Health Care Options.
Minimum Qualifications:
Education: Completion of the following:
- Minimum of a Master’s Degree in Social Work (MSW) from an accredited School of Social Work by the Council on Social Work Education (CSWE)
Experience: Candidates must have the following experience:
- Minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.
- Military experience is not required but is a plus.
Security:
- National Background Investigation
- Must be a US Citizen
- Must have reliable transportation to and from work site
License/Certification and Training:
- Current, full, active, and unrestricted license to practice as a Licensed Independent Clinical Social Worker (LICSW)
- Current Basic Cardiac Life Support (BCLS) from American Heart Association.
Schedule:
- Monday to Friday
- 7:30 AM to 4:30 PM
