Job Description:

• Identify Patients that need special medical and education needs/management.
• Conducts assessments of family members of regular component Air Forse sponsors to determine if special medical and/or educational conditions exist that require Exceptional Family Member Program enrollment.
• Comprehensively collects patient information and medical status, and continued evaluation of an established care plan.
• Collaborates with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of care.
• Shall be integrally involved in the Family Member Relocation Clearance process.
• Demonstrate a working knowledge of professional standards and ethics regarding the delivery of clinical social work services.
• Ensures that sponsors are provided all necessary documents for enrollment in TRICARE, Extended Health Care Options.

Minimum Qualifications:
Education: Completion of the following:

• Minimum of a Master’s Degree in Social Work (MSW) from an accredited School of Social Work by the Council on Social Work Education (CSWE)

Experience: Candidates must have the following experience:

• Minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.
• Military experience is not required but is a plus.

Security:

• National Background Investigation
• Must be a US Citizen
• Must have reliable transportation to and from work site

License/Certification and Training:

• Current, full, active, and unrestricted license to practice as a Licensed Independent Clinical Social Worker (LICSW)
• Current Basic Cardiac Life Support (BCLS) from American Heart Association.

• Monday to Friday
• 7:30 AM to 4:30 PM
